Being such a huge fan of the American version I couldn't see how added footage would improve it, but I was wrong. See the extended (original) version. Far superior (though, at times, far creepier).
If you read the giant manga you'll have only a marginally deeper understanding of the events in this film. Either way, once you start watching it, it's difficult to stop.
Exploitative and over-the-top? Yes. But the craft that went into making this movie (all old-school movie effects pre-1960) makes it worth it. Also, I first saw it as a kid and it was one of the movies that stuck with me.
If this movie hadn't been made, we wouldn't have gotten that massive 1980s teen slasher movie genre. Still trying to decide if that's a good thing or a bad thing.
The theatrical version is so-so. The director's cut is absolute genius and one of the best historical epics ever made. Seriously, it's up there with Lawrence of Arabia and Spartacus.

It's probably not as good as I remember it when I saw it as a kid. I do remember it being very slow. Nevertheless, it was one of the movies that got me into anime in the first place.
This movie is thoroughly flawed. The structure is very strange, everyone talks FOREVER, and the characters are off, but I just can't seem to care about all that. Plus, it's got one of the best car chases ever filmed.
I remember my parents not wanting me to see this when I was a kid because it's disturbing and not for children. This is the very thing that made me want to see it. So when they were out I snuck a watch of my dad's copy. Then I saw they were right.
